Maintenance Agreement

Any project involving the acquisition, retrofit, or modification of existing public/private property resulting in public ownership or management of property, structures, or facilities, must sign the agreement prior to submitting their application.

Note: Projects including the retrofit, elevation or other modification to private property where ownership will remain private need not complete this form.
