Each applicant must complete the FEMA 75-5 form (http://training.fema.gov/Apply/75-5.pdf) and forward it directly to Deborah Hanley at (850)488-6250 (fax) or deborah.hanley@EM.MyFlorida.com for consideration in the nomination process. Your application must be accompanied by the following prerequisite course certificates: IS-271, IS-324, and G363 These documents must be faxed or emailed prior to the course for State approval.

When a course is not filled by individuals from the host area six weeks before the course date, applicants will be accepted from any other area of the state on a first come, first served basis.
