Each applicant must complete the FEMA 75-5 form (http://training.fema.gov/Apply/75-5.pdf) and forward it directly to Deborah Hanley at (850)488-6250 (fax) or deborah.hanley@EM.MyFlorida.com for consideration in the nomination process. Your application must be accompanied by the following prerequisite course certificates: IS-271, IS-324, and G363 These documents must be faxed or emailed prior to the course for State approval.

director and the FDEM Training administrators. Individuals from the host county and area where
the course is being held have first priority for acceptance.
When a course is not filled by individuals from the host area six weeks before the course date,
applicants will be accepted from any other area of the state on a first come, first served basis.
