It is critical that all residents have a place in case of a disaster. It is even more important for people with access and functional needs to take the proper steps to prepare themselves in the event of a disaster or other emergency.

The best way to be prepared is to be informed. To better assist you in creating a strong emergency plan, reach out to resources in your community and learn of all the options available to you.

HOME INFORMATION

Building your Special Needs Emergency Plan is the first step in preparing for all types of disasters. First, in order to provide you with a plan specific to your address, basic contact information is required.

Section 252.905, Florida Statutes declares any information furnished by a person or a business to the Florida Division of Emergency Management for the purpose of being provided assistance with emergency planning is exempt from 119.07(1) and s. 24(a), Art. I of the State Constitution.

Therefore, information provided through this website for the purposes of building your emergency plan is exempt from public records requests made of the Division and is only used for the express purpose of allowing visitors to this website to build and maintain plans.

 

HOUSEHOLD MEMBERS

A Special Needs Emergency Plan should include items for each of your family members. In order to calculate the correct amount of items, information is required for each family member. The information provided will be used in building your Special Needs Emergency Plan and will not be maintained on the system.

 

PETS

When building your Special Needs Emergency Plan it is important to consider your service animals and their needs during a disaster. There is a difference between a service animal and a family pet. If you have a family pet that will need to evacuate with you in an emergency, please contact your county emergency management to identify a pet friendly shelter. Please enter the number of animals in the appropriate category.

 

OUT OF TOWN CONTACT

You should also identify an out-of-town contact that all family members know to reach should you become separated during an emergency. Family members will need to contact the pre-identified person in an emergency to report their location and that they are safe.

 

MEETING LOCATION

A Special Needs Emergency Plan should include a location away from home for family members to meet in case of a disaster and your home is inaccessible. Identify a location for your family to meet, preferably one close to your community. Be sure to discuss this location with all family members so that everyone understands where to meet in an emergency.

 


FUNCTIONAL OR ACCESS NEEDS

Do you, or a family member, have a disability, chronic health issue, or a medical situation that will require assistance in the event of a disaster?

  • Assistance to evacuate during a disaster
  • Assistance with daily activities (bathing, eating, toileting, dressing)
  • Assistance with dressing changes or wound care
  • Assistance with medication
  • Electricity for life supporting medical equipment
  • Observation by a health professional
  • Oxygen therapy

ASSISTANCE REQUIRED

Do you, or a family member, currently receive services for daily living including, but not limited to, feeding (Meals on Wheels), transportation (Dial-A-Ride; Neighborly Senior Services), durable medical supplies, home health services, personal care, dialysis, hospice, or other assistance?

YOUR PLAN IS COMPLETE

Congratulations. You have successfully created your disaster plan.

The information you provided while developing your Special Needs Emergency Plan indicates you qualify for the Florida Special Needs Registry. Once you have downloaded your customized Special Needs Emergency Plan, click on the box to the right “Florida Special Needs Registry” to be directed to the registry. You can access the Florida Special Needs Registry from the front page of FLGetAPlan.com to update your information.
